What adult trials actually show

A randomized study in healthy adults tested 2’-FL and/or LNnT at 5, 10 and 20 g/day for two weeks. HMO supplementation was generally well tolerated and produced substantial increases in Bifidobacterium along with broader microbiota shifts.

That supports a microbiota effect. It does not by itself establish symptom benefit.

IBS: microbiota change without symptom improvement

A randomized IBS study tested 5 g or 10 g/day of a 2’-FL/LNnT combination for four weeks. The 10 g group increased fecal bifidobacteria, but overall and individual gastrointestinal symptom severity did not differ significantly from placebo.

This is particularly important because retail HMO marketing often implies that changing the microbiome equals relieving IBS. The trial shows why those are separate endpoints.

Older adults: another useful negative primary endpoint

A 2025 randomized trial in healthy older adults tested 1 g and 5 g/day 2’-FL. Bifidobacterium increased and several metabolic/hormonal measures changed, but the study’s primary cytokine-response endpoint was not met.

Again: interesting biology, but not a universal clinical win.
