Ringworm is a fungal infection, not a worm
Ringworm, or tinea corporis, is a dermatophyte infection of body skin. It often produces an itchy, scaly, expanding patch with a more active edge, but the classic ‘ring’ is not present in every case.
Typical treatment approach
For limited skin disease, over-the-counter topical antifungals such as terbinafine, clotrimazole or miconazole are commonly used. Follow the individual product label for where it can be used, how often to apply it and how long to continue.
Treat the entire affected area and a small margin of surrounding skin rather than only the most visibly red edge.
Why steroid-only treatment can backfire
Topical corticosteroids reduce inflammation and itching but do not kill dermatophytes. Used alone on ringworm, they can suppress the visible inflammation while fungus continues to grow, creating a less typical pattern sometimes called tinea incognito.
Scalp ringworm is different
Tinea capitis affects the scalp and hair shafts. Topical creams alone generally do not reach infected hair follicles adequately, so clinician-prescribed oral antifungal medication is typically required.
Reduce spread and reinfection
Ringworm can spread through direct skin contact and contaminated objects. Launder clothing, bedding and towels that contact the affected area, avoid sharing personal items, keep affected skin clean and dry, and consider whether household members or pets have suspicious lesions.
Conditions that can mimic ringworm
Nummular eczema, psoriasis, pityriasis rosea, granuloma annulare and contact dermatitis can create circular or scaly patches. If the pattern is unusual or a correctly used antifungal does not help, diagnosis is more useful than cycling through multiple products.
When to seek medical care
Get evaluated for scalp or beard involvement, widespread disease, repeated recurrences, signs of bacterial infection, uncertain diagnosis, immune suppression, or failure to improve with an appropriate course of topical treatment.
